Community Support

At Eley Care we will work with you to tailor a support plan that is centred around the goals made in your NDIS plan. This could include, support that is based in the Community. We will ensure you are properly matched with a support worker that can best help you fulfil these goals and truly be part of the community. The type of community support we offer are:

• Support to participate in community event/group

• Support with any personal care while in the community

• Support teaching life skills in the community

• Cash handling support in the community
